Meal Request Form - Week of March 30

Please use this form to order breakfast and lunch for the week of March 30th, Monday-Thursday. To help us better prepare meals, list your students names below.

A grab & go lunch will be available for pick-up Monday 11:30 a.m. to 12:30 p.m. A grab & go breakfast/lunch combination will then be available EVERYDAY following thru Thursday. Meals will be served only thru vehicle window lining up on the street just west of the school cafeteria.

Please do not get out of your vehicle or come inside the building, meals will be handed to you. (Drive through style.)

Please have this form filled out by noon on Thursday, March 26.

This form is for the week dated above, a new form will be required for any following week(s).
